The Peyer's patch B-cell IgA secretion pathway is a specialized immune axis within the gut-associated lymphoid tissue (GALT) responsible for generating mucosal IgA antibodies (NIH, 2016). Peyer's patches serve as the primary inductive sites where B cells encounter antigens and undergo class switch recombination to the IgA isotype, a process regulated by cytokines such as TGF-beta, BAFF, and APRIL (NIH, 2018). Following activation, these B cells migrate to the intestinal lamina propria to become IgA-secreting plasma cells (StatPearls, 2023). Dysregulation of this pathway, particularly the overproduction of galactose-deficient IgA1 (Gd-IgA1), is a hallmark of IgA nephropathy, leading to the formation of immune complexes that deposit in the kidneys (NIH, 2018). Therapeutic strategies targeting this pathway include the use of targeted-release formulations of budesonide to suppress local B-cell activity in the ileum (NIH, 2018). Additionally, monoclonal antibodies and fusion proteins that inhibit BAFF and APRIL are being developed to prevent IgA class switching and plasma cell survival (NIH, 2023). These interventions aim to reduce the production of pathogenic IgA1 and mitigate subsequent renal damage in patients with IgA nephropathy (NIH, 2018). The pathway is also a focus for mucosal vaccine development, where enhancing IgA secretion can provide better protection against enteric pathogens (Frontiers, 2023).
The pathway is targeted by suppressing B-cell activation and class switch recombination through glucocorticoid receptor agonism or the inhibition of B-cell survival factors like BAFF and APRIL (NIH, 2018; NIH, 2023).
