Overview

Phosphodiesterase 3A (PDE3A) is a key enzyme responsible for the hydrolysis of cyclic adenosine monophosphate (cAMP) in platelets, vascular smooth muscle, and cardiac tissue (UniProt: P17174). It is often referred to as the cGMP-inhibited phosphodiesterase because cGMP acts as a competitive inhibitor of its cAMP-hydrolyzing activity. In platelets, PDE3A is the primary regulator of cAMP; increasing cAMP levels through PDE3A inhibition prevents platelet activation and aggregation by activating protein kinase A (PKA) (PubMed: 11511511). This enzyme is a major therapeutic target for cardiovascular conditions, particularly peripheral arterial disease and intermittent claudication. Drugs such as cilostazol act as selective PDE3 inhibitors, providing both antiplatelet and vasodilatory effects (StatPearls: Cilostazol). However, because PDE3A is also involved in cardiac contractility, its inhibition can lead to cardiovascular side effects such as palpitations and tachycardia (PubMed: 15130913).

Mechanism of action

PDE3A inhibitors bind to the catalytic site of the enzyme, preventing the hydrolysis of cAMP to 5'-AMP. In platelets, the resulting rise in cAMP activates protein kinase A (PKA), which phosphorylates substrates like VASP, leading to the inhibition of GPIIb/IIIa receptor activation and reduced platelet aggregation (StatPearls: Cilostazol; PubMed: 11511511). In vascular smooth muscle, elevated cAMP levels lead to the phosphorylation of myosin light chain kinase, resulting in vasodilation (DrugBank: DB01166).
