Plasma exchange—also known as plasmapheresis or therapeutic plasma exchange—is an extracorporeal blood purification technique designed to remove pathogenic substances such as autoantibodies, immune complexes, paraproteins, cryoglobulins, lipids, and certain toxins from the patient's plasma. The process involves separating whole blood into its cellular components and plasma using centrifugation or filtration. The plasma, which contains these harmful molecules driving disease processes in autoimmune and some neurological conditions, is discarded or treated before being replaced with substitute solutions containing albumin or fresh frozen plasma. The remaining blood cells are then returned to the patient along with this replacement fluid. This approach provides rapid reduction in circulating levels of pathological antibodies that contribute to acute exacerbations in diseases like Guillain-Barré syndrome, myasthenia gravis crisis, chronic inflammatory demyelinating polyneuropathy (CIDP), certain renal pathologies including transplant rejection episodes mediated by donor-specific antibodies, among others. It can be life-saving when conventional immunosuppressive therapies are insufficiently fast acting. While highly effective for short-term control of severe autoimmune flares by removing causative agents from circulation, it does not address underlying production mechanisms—thus repeated treatments may be necessary. Notable safety concerns include allergic reactions to replacement fluids/columns, hypotension during treatment, risk for infection at vascular access sites, and potential depletion of beneficial proteins/clotting factors. Importantly: "Pathogenic antibodies removal via plasma separation" describes a therapeutic procedure, not a discrete molecular target such as an enzyme or receptor. Therefore it should not be classified as a canonical drug target but rather as an interventional modality within clinical immunology/hematology.
The mechanism is physical removal of circulating pathogenic substances such as autoantibodies, immune complexes, paraproteins, and toxins from the patient’s plasma using extracorporeal separation techniques.
