Proinsulin-specific CD8+ T cells are a subset of cytotoxic T lymphocytes that recognize peptides derived from proinsulin, the precursor molecule to insulin, presented by HLA class I molecules on pancreatic beta cells. These autoreactive T cells play a central role in the pathogenesis of type 1 diabetes (T1D) by mediating immune destruction of insulin-producing beta cells. Strategies aimed at reducing their frequency or function (e.g., plasmid-based tolerization) have shown promise for preserving residual C-peptide secretion (a marker for endogenous insulin production).
