Lubricin, also known as Proteoglycan 4 (PRG4), is a mucin-like glycoprotein crucial for maintaining joint health and function. It acts as a boundary lubricant, significantly reducing friction and wear on articular cartilage surfaces and preventing synovial cell adhesion and proliferation. Beyond its lubricating properties, lubricin exhibits anti-inflammatory effects by antagonizing Toll-like receptors 2 and 4 (TLR2/TLR4) and inhibiting inflammatory pathways. Deficiencies in lubricin are implicated in various conditions, including osteoarthritis, camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome, dry eye, and sepsis. As a therapeutic target, recombinant human PRG4 (rhPRG4) and lubricin mimics are being investigated to supplement or restore its function, particularly in osteoarthritis and post-traumatic joint injuries. Biomarkers such as lubricin levels themselves, IL-6, and urinary CTXII are used for diagnosis and monitoring disease progression. Challenges in targeting lubricin include the delivery of this large glycoprotein and its potential degradation, driving research into small molecule enhancers and mimics.
Recombinant lubricin and lubricin mimics function by providing boundary lubrication, reducing friction, preventing cell adhesion, protecting cartilage surfaces, and exerting anti-inflammatory effects through antagonism of Toll-like receptors 2 and 4 (TLR2/TLR4), blocking NF-κB translocation, and inhibiting the NLRP3 inflammasome. Drugs like Pioglitazone aim to increase the endogenous production of lubricin.
