Rabphilin 3A like (RPH3AL) is a protein primarily recognized for its regulatory role in calcium-ion-dependent exocytosis in endocrine and exocrine cells and its involvement as a putative tumor suppressor. It encodes a protein playing a direct regulatory role in calcium-ion-dependent exocytosis in both endocrine and exocrine cells, notably involved in insulin secretion by pancreatic cells. RPH3AL functions as a Rab GTPase effector and is recognized as a putative tumor suppressor. Genetic alterations such as missense mutations or reduced expression are associated with increased risk, recurrence, and severity of colorectal and breast cancers, marking RPH3AL as a clinically relevant biomarker for prognosis in these settings. Despite its key role in regulated exocytosis and tumor suppression, it is not directly targeted by approved therapeutic agents and requires further research for potential clinical targeting. RPH3AL is a Rab3 effector, distinct from Rabphilin-3A (missing the C2 domain). Functional roles include calcium-dependent vesicle exocytosis and tumor suppression. Clinical relevance lies in mutations/decreased expression indicating poor prognosis in certain cancers. No drug targeting has been identified, but variants can be used as biomarkers for patient stratification in oncology.
