CD45RA is a high-molecular-weight isoform of the Receptor-type tyrosine-protein phosphatase C (PTPRC), a transmembrane protein essential for the activation of T and B lymphocytes. It functions by modulating the phosphorylation state of Src-family kinases, thereby regulating the sensitivity of the T-cell receptor (TCR) signaling pathway (UniProt P08575). In the context of allogeneic hematopoietic stem cell transplantation, CD45RA is a critical target for graft engineering to prevent Graft-versus-Host Disease (GvHD). Because CD45RA is predominantly expressed on naive T cells—the primary drivers of GvHD—its selective depletion from donor leukocytes allows for the preservation of CD45RO+ memory T cells (PubMed: 24928257). This strategy aims to provide the recipient with immediate T-cell mediated immunity against infections and leukemia while significantly reducing the risk of systemic host tissue attack (PubMed: 32155513).
