Rh D antigen is a transmembrane protein expressed on erythrocytes, encoded by the RHD gene. It is responsible for the D antigen of the Rh blood group—one of the most clinically important blood group antigens due to its role in hemolytic disease of the newborn and transfusion reactions. RhD forms part of a complex with RhAG and RhCE proteins in the red cell membrane, maintaining membrane integrity and possibly participating in transport mechanisms, though no definitive physiological function is established for RhD itself. The protein is a major immunogen for humans; anti-D antibody formation occurs readily in RhD-negative individuals exposed via transfusion or pregnancy to RhD-positive erythrocytes. Partial and weak D variants arise from gene conversions or point mutations, influencing clinical management in transfusion and obstetric medicine.
Anti-D immunoglobulin binds Rh D antigen on fetal/neonatal/blood donor erythrocytes, prevents maternal sensitization (formation of anti-D antibodies), and blocks immune-mediated hemolysis
