Overview

Saponin-based adjuvants are a group of naturally derived or synthetic triterpene glycosides commonly used to boost the immune response in vaccines. They do not represent a singular molecular target but instead enhance immune system stimulation by activating antigen-presenting cells through both general (membrane permeability alteration, cytokine induction) and potentially specific (receptor-mediated) mechanisms. Saponin adjuvants are present in several approved human vaccines (including Shingrix, Mosquirix, and the Novavax COVID-19 vaccine), where they promote both humoral (antibody-mediated) and cellular (T cell-mediated) immunity. The best-studied saponin adjuvant component is QS-21 (a highly purified saponin from Quillaja saponaria), often formulated with other immunostimulants (e.g., MPLA in AS01). Saponins may interact with innate immune receptor pathways such as the NLRP3 inflammasome and enhance antigen cross-presentation, but no single dedicated protein or receptor has been universally identified as the “pharmacological target” of these adjuvants.

Mechanism of action

Saponins interact with antigen-presenting cells (APCs), possibly by binding to or integrating into cellular membranes, enhancing antigen presentation, and cross-presentation. Some saponins (e.g., QS-21) may act through receptor-mediated and non-receptor-mediated processes, including potential specific interactions with T cell surface proteins, NLRP3 activation in APCs, and cholesterol-dependent membrane effects. Induction of pro-inflammatory cytokines (e.g., IL-1β, IL-18, IFN-γ), enhanced costimulatory molecule expression (e.g., CD86, CD40), and recruitment of neutrophils and monocytes. Promoting the development of strong Th1 and CTL responses, important for viral and tumor immunity.
