Serine beta-lactamases of Ambler class C, commonly referred to as AmpC beta-lactamases or cephalosporinases, are enzymes produced by certain bacteria that confer resistance to a broad range of beta-lactam antibiotics. These enzymes hydrolyze the beta-lactam ring found in these antibiotics, rendering them ineffective against bacterial cell wall synthesis. AmpC enzymes are generally not inhibited by traditional β-lactamase inhibitors such as clavulanic acid or tazobactam. They are significant causes of hospital-acquired infections due to their ability to resist many commonly used β-lactams. Third-generation cephalosporins should generally be avoided when treating infections caused by organisms known for inducible AmpC production, regardless of initial susceptibility test results.
