Sesame allergens are a group of proteins derived from the seeds of Sesamum indicum that elicit IgE-mediated hypersensitivity reactions in sensitized individuals [WHO/IUIS]. The most clinically significant allergens include the 2S albumins (Ses i 1 and Ses i 2), the 7S globulin (Ses i 3), and the 11S globulins (Ses i 6 and Ses i 7), which primarily function as seed storage proteins providing nitrogen and sulfur for the developing embryo [UniProt]. Other identified allergens include Ses i 4 and Ses i 5, which are oleosins responsible for stabilizing lipid droplets within the seed [PubMed: 11485451]. These proteins are characterized by high stability against thermal denaturation and proteolytic digestion, which allows them to maintain their allergenicity through food processing and digestion [Adatia et al., 2017]. In the human body, these allergens are not therapeutic targets but rather exogenous triggers that cross-link specific IgE on mast cells and basophils, leading to the release of inflammatory mediators. While management traditionally focuses on strict avoidance and emergency epinephrine, sesame allergen extracts are utilized in clinical trials and specialized practice for oral immunotherapy (OIT) to induce desensitization [ClinicalTrials.gov].
