Overview

Short stature homeobox protein 2 (SHOX2) pre-messenger RNA is the primary transcript of the SHOX2 gene, which encodes a homeodomain-containing transcription factor essential for embryonic development (UniProt O60902). SHOX2 is a master regulator of the cardiac conduction system, specifically the sinoatrial node (SAN), where it maintains the pacemaker program by repressing Nkx2-5 and activating Tbx3 (PubMed: 21852317). Beyond its cardiac role, SHOX2 is involved in limb development and craniofacial morphogenesis. In clinical oncology, SHOX2 pre-mRNA and its gene are significant due to the high frequency of DNA methylation in the SHOX2 promoter region, which serves as a sensitive and specific biomarker for the diagnosis of lung cancer and other malignancies (PubMed: 23430945). While no drugs targeting SHOX2 pre-mRNA are currently FDA-approved, it represents a potential target for RNA-based therapeutics, such as antisense oligonucleotides (ASOs), designed to modulate splicing or expression in patients with sinus node dysfunction or specific cancers. Therapeutic intervention must be carefully managed due to the gene's critical role in maintaining normal cardiac rhythm and its broad developmental impact.
