Signal transducer and activator of transcription 5B (STAT5B) is a critical transcription factor that mediates cellular responses to a wide array of cytokines, interleukins, and growth hormones [1, 2]. Upon the binding of ligands to their respective receptors, STAT5B is phosphorylated by Janus kinases (JAKs), leading to its dimerization and translocation into the nucleus where it binds to specific DNA sequences to initiate gene transcription [1, 5]. This protein is essential for regulating biological processes such as cell proliferation, differentiation, and survival, particularly within the hematopoietic and immune systems [3]. In clinical pathology, gain-of-function mutations in STAT5B, such as the N642H variant, are potent oncogenic drivers in various hematologic malignancies, including T-cell prolymphocytic leukemia and large granular lymphocytic leukemia [4]. Conversely, germline loss-of-function mutations result in a syndrome characterized by growth hormone insensitivity, short stature, and severe immune dysregulation [1, 2]. While direct small-molecule inhibitors of the STAT5B SH2 domain are currently under intensive investigation, the pathway is most commonly modulated in clinical practice through the use of upstream JAK inhibitors like ruxolitinib [3, 5]. Therapeutic targeting of STAT5B presents challenges due to its high structural homology with STAT5A, necessitating the development of highly selective agents to minimize off-target toxicities [3].
Direct inhibition of the STAT5B SH2 domain to prevent dimerization and DNA binding, or indirect inhibition via the blockade of upstream Janus kinases (JAK1/2/3) to prevent STAT5B phosphorylation.
