The Sodium-hydrogen exchanger (NHE) is a family of transmembrane transporters, primarily belonging to the Solute Carrier 9 (SLC9) family, that mediate the electroneutral exchange of extracellular sodium ions for intracellular protons. These exchangers play a critical role in maintaining intracellular pH, regulating cell volume, and facilitating sodium reabsorption in the kidneys and gastrointestinal tract (UniProt, 2024). There are several isoforms, with NHE1 being ubiquitously expressed and NHE3 being localized to the apical membranes of renal and intestinal epithelia (NCBI, 2023). In cardiovascular disease, overactivity of NHE1 during ischemia-reperfusion injury leads to calcium overload and cell death, making it a target for cardioprotective agents like cariporide (PubMed, 2021). Conversely, inhibition of NHE3 by drugs such as tenapanor reduces intestinal sodium absorption, which is utilized to treat irritable bowel syndrome with constipation and hyperphosphatemia in chronic kidney disease (FDA, 2022). Dysregulation of NHE isoforms is also implicated in cancer progression, where they contribute to the acidic tumor microenvironment that promotes invasion and metastasis (Nature Reviews Cancer, 2020).
