Overview

The stratum corneum (SC) lipid matrix is the primary physical barrier of the skin, composed of ceramides, cholesterol, and free fatty acids organized into highly ordered lamellar bilayers [1]. This extracellular matrix fills the spaces between corneocytes, creating a brick and mortar structure that is essential for preventing excessive transepidermal water loss (TEWL) and maintaining cutaneous hydration [2]. Beyond hydration, the lipid matrix serves as a critical defense against environmental insults, including chemical irritants, allergens, and microbial pathogens [3]. In diseases such as atopic dermatitis and psoriasis, the composition and organization of these lipids are significantly altered, leading to a compromised barrier and increased susceptibility to inflammation [4]. Therapeutic strategies targeting this structure include the application of barrier-repair emollients containing physiological lipids to restore the lamellar organization and occlusive agents to provide temporary protection [5].
