Overview

The Streptococcus mutans biofilm extracellular matrix (ECM) is a complex, self-produced scaffold of extracellular polymeric substances (EPS) that anchors bacteria to the tooth surface and provides structural integrity to dental plaque (Koo et al., 2013, PubMed: 23632761). Composed primarily of exopolysaccharides (glucans and fructans), extracellular DNA (eDNA), and proteins, the matrix acts as a protective barrier against antimicrobial agents and host immune defenses (Flemming and Wingender, 2010, Nature Reviews Microbiology). S. mutans utilizes enzymes called glucosyltransferases to synthesize these glucans from dietary sucrose, which facilitates the formation of highly acidic microenvironments within the biofilm (Bowen and Koo, 2011, Caries Research). These localized acidic zones lead to the demineralization of tooth enamel, the primary cause of dental caries (Selwitz et al., 2007, The Lancet). Therapeutic interventions targeting the matrix aim to disrupt its assembly, degrade its structural components, or inhibit the enzymes responsible for its synthesis to restore oral health (Koo et al., 2017, Journal of Dental Research). By weakening the matrix, these treatments enhance the efficacy of traditional antimicrobials and prevent the progression of tooth decay.
