Overview

The Synthetic T-cell Antigen Receptor (STAR) is an engineered chimeric receptor designed to combine the high-affinity antigen recognition of an antibody with the natural signaling efficiency of the T-cell receptor (TCR) complex [1]. It is typically constructed by fusing antibody-derived variable regions (such as an scFv or VHH) to the constant regions of the TCR alpha and beta chains, which allows the STAR receptor to spontaneously integrate into the endogenous CD3 signaling complex [1,3]. This structural mimicry of the natural TCR enables STAR receptors to utilize the full suite of intracellular signaling molecules, often resulting in superior sensitivity to low-density antigens and improved metabolic profiles compared to traditional Chimeric Antigen Receptors (CARs) [1,2]. STAR-based therapies are primarily developed for oncology, where they are engineered into T-cells to target specific tumor-associated antigens (TAAs) such as BCMA or CD19 [2,3]. By leveraging the natural T-cell machinery, STAR-T cells aim to provide a more potent and durable anti-tumor response in both liquid and solid malignancies [1,3].
