Systemic fluid, electrolyte, and acid–base balance is a complex physiological state maintained through the integrated function of the renal, respiratory, and endocrine systems (StatPearls, 2023). It involves the precise regulation of total body water, the concentration of essential ions such as sodium and potassium, and the maintenance of arterial blood pH within a narrow range (NIH, 2022). While not a single molecular target, this homeostatic balance is the primary therapeutic objective for numerous drug classes, including diuretics, ACE inhibitors, and vasopressin antagonists (Merck Manual, 2023). These pharmacological agents typically act on specific transporters or receptors in the kidney to modulate solute and water excretion (PubMed, 2021). Disruptions in this balance are central to the pathology of conditions like heart failure, chronic kidney disease, and metabolic disorders. Consequently, monitoring serum electrolytes and acid-base status is critical for ensuring patient safety and therapeutic efficacy in clinical practice (StatPearls, 2023).
