Overview

The T cell receptor beta chain variable region (TRBV) is the highly diverse, N-terminal portion of the TCR beta chain that, in conjunction with the alpha chain, forms the antigen-binding site of the T cell receptor (UniProt: P01850). Its primary biological function is the recognition of specific peptide antigens presented by Major Histocompatibility Complex (MHC) molecules, which is the central event in T cell-mediated immunity (PubMed: 25142320). In clinical pathology, the TRBV region is the specific target of bacterial superantigens, such as Toxic Shock Syndrome Toxin-1 (TSST-1), which cross-link the TCR to MHC II molecules to trigger a massive, systemic cytokine storm (PubMed: 15153528). Furthermore, the restricted expression of specific TRBV families is a diagnostic hallmark of T-cell malignancies, including T-cell large granular lymphocyte (LGL) leukemia and cutaneous T-cell lymphoma (PubMed: 8530554). Therapeutic strategies currently under investigation include the use of monoclonal antibodies and TCR-engineered T cells that target specific TRBV segments to selectively eliminate malignant or autoreactive T-cell clones while preserving the overall immune repertoire (PubMed: 31534206). This approach offers a more precise alternative to pan-T-cell depletion, potentially reducing the risk of broad immunosuppression.

Mechanism of action

Superantigens (e.g., TSST-1) act as molecular bridges between the TRBV region and MHC class II molecules, bypassing normal antigen processing to induce massive T-cell activation (PubMed: 15153528). Therapeutic monoclonal antibodies target specific TRBV families to induce depletion of malignant or pathogenic T-cell clones via antibody-dependent cellular cytotoxicity (ADCC) or complement-dependent cytotoxicity (CDC) (PubMed: 31534206).
