Overview

The T-cell receptor-CD3 (TCR-CD3) complex is a multi-subunit transmembrane protein assembly essential for the adaptive immune response. It consists of an antigen-binding TCR heterodimer (alpha/beta or gamma/delta) associated with four invariant CD3 signaling chains: epsilon, delta, gamma, and zeta [1, 4]. The complex functions by recognizing peptide antigens presented by Major Histocompatibility Complex (MHC) molecules, which triggers intracellular signaling via Immunoreceptor Tyrosine-based Activation Motifs (ITAMs) located on the CD3 subunits [2]. This signaling is crucial for T-cell maturation, activation, and the execution of effector functions against pathogens and tumors. In clinical practice, the TCR-CD3 complex is a major target for immunotherapy; bispecific antibodies (e.g., Blinatumomab) bind CD3 to physically link T cells to cancer cells, inducing targeted lysis [3]. Additionally, anti-CD3 antibodies like Teplizumab are used to modulate T-cell activity in autoimmune conditions such as Type 1 Diabetes by inducing a state of partial exhaustion or regulatory T-cell promotion [5].

Mechanism of action

The TCR-CD3 complex is targeted by monoclonal antibodies to either induce immunosuppression through T-cell depletion and signaling modulation or to redirect T-cell cytotoxicity against tumor cells via bispecific engagement [3, 5].
