Overview

The Tissue Factor-Factor VIIa (TF-FVIIa) complex, also known as the extrinsic tenase, is the primary initiator of the blood coagulation cascade (Mackman, 2009). It forms when Tissue Factor (TF), a transmembrane glycoprotein, is exposed to circulating Factor VIIa following vascular injury or inflammatory stimuli, assembling on anionic phospholipid surfaces to facilitate enzymatic activity (Butenas et al., 2000). This complex activates Factors X and IX, leading to a burst of thrombin and the formation of a fibrin clot. Beyond hemostasis, TF-FVIIa signaling through protease-activated receptors (PARs) plays a significant role in angiogenesis, inflammation, and tumor progression (Rao & Pendurthi, 2005). Pathologically, the complex is central to the development of thrombosis and is overexpressed in many solid tumors, where it contributes to metastasis (Versteeg et al., 2013). Therapeutic strategies include direct inhibitors to prevent clotting and antibody-drug conjugates, such as tisotumab vedotin, which target TF-expressing cancer cells (FDA, 2021). However, targeting this complex carries a high risk of bleeding due to its essential role in normal physiological clotting.
