Overview

Tumor necrosis factor ligand superfamily member 4, commonly known as OX40L or CD252, is a type II transmembrane protein that serves as the cognate ligand for the OX40 receptor (CD134). It is primarily expressed on activated antigen-presenting cells (APCs) such as dendritic cells, B cells, and macrophages, as well as on vascular endothelial cells. The binding of OX40L to OX40 provides a potent co-stimulatory signal to T cells, which is essential for their survival, proliferation, and the production of pro-inflammatory cytokines. This interaction plays a pivotal role in the transition from innate to adaptive immunity and the formation of long-term memory T cells. In various inflammatory and autoimmune diseases, the OX40/OX40L pathway is often overactive, leading to chronic T-cell mediated tissue damage. For instance, in atopic dermatitis, OX40L-mediated signaling promotes Th2 cell responses that drive skin inflammation. Consequently, OX40L has emerged as a significant therapeutic target. Monoclonal antibodies designed to block OX40L, such as amlitelimab, are being developed to treat moderate-to-severe inflammatory conditions by dampening the excessive T-cell activity without causing broad depletion of the immune system. In oncology, while the focus is often on OX40 agonists, the regulation of OX40L remains a key area of interest for modulating the tumor microenvironment.
