Overview

The urothelial glycosaminoglycan (GAG) layer is a thin, highly hydrophilic coating on the luminal surface of the bladder urothelium, primarily composed of molecules such as hyaluronic acid, heparan sulfate, and chondroitin sulfate (Hurst RE, 1994, PMID: 8144946). Its primary biological function is to serve as a non-specific barrier that prevents the adherence of bacteria and protects the underlying bladder wall from irritating solutes, such as potassium and toxins, present in the urine (Parsons CL, 2007, PMID: 17431816). In pathological states like interstitial cystitis/bladder pain syndrome (IC/BPS), the GAG layer is often compromised or "leaky," allowing urinary constituents to penetrate the urothelium and trigger chronic inflammation and sensory nerve activation (StatPearls, NBK470312). Therapeutic strategies targeting this layer, known as GAG replenishment therapies, aim to restore this protective barrier using exogenous GAG analogs. Drugs such as pentosan polysulfate sodium (oral) and intravesical instillations of hyaluronic acid or chondroitin sulfate are used to alleviate symptoms by reinforcing the bladder's natural defense mechanism. While generally safe, long-term use of oral pentosan polysulfate has been linked to specific safety concerns like pigmentary maculopathy (FDA Label, Elmiron). This target is unique in urology as it focuses on structural restoration of the extracellular matrix rather than direct receptor modulation.
