Overview

The vaginal extracellular matrix (ECM) is a dynamic and complex network of macromolecules, including collagen, elastin, and proteoglycans, that provides essential structural support and mechanical properties to the vaginal wall (National Institutes of Health, 2023). It plays a critical role in maintaining tissue elasticity and tensile strength, which are vital for normal pelvic floor function and reproductive health (PubMed, PMID: 15749514). Pathological remodeling of the vaginal ECM, often involving an imbalance between matrix synthesis and degradation by matrix metalloproteinases (MMPs), is a hallmark of conditions such as pelvic organ prolapse and vaginal atrophy (PubMed, PMID: 22403107). Therapeutic interventions often target the ECM by utilizing estrogens or selective estrogen receptor modulators (SERMs) to stimulate the production of collagen and elastin, thereby restoring tissue integrity (PubMed, PMID: 28813331). Additionally, regenerative medicine approaches focus on using decellularized vaginal ECM or synthetic scaffolds to promote tissue repair and functional recovery in patients with pelvic floor disorders (PubMed, PMID: 30145564).
