Overview

Volatile sulfur compounds (VSCs) are a group of gases, primarily hydrogen sulfide, methyl mercaptan, and dimethyl sulfide, that are the principal contributors to oral malodor, or halitosis (Tangerman & Winkel, 2007). These compounds are generated through the metabolic activity of anaerobic Gram-negative bacteria, such as Porphyromonas gingivalis and Treponema denticola, which degrade sulfur-containing amino acids like cysteine and methionine found in saliva, gingival crevicular fluid, and food debris (Persson et al., 1990). While primarily known for their foul odor, VSCs are also pathologically significant as they can increase the permeability of the oral mucosa and inhibit the synthesis of collagen and non-collagenous proteins, thereby contributing to the progression of periodontal diseases (Ng & Tonzetich, 1984). Pharmacological management does not involve traditional drug-receptor interactions but instead utilizes chemical neutralizers like zinc ions, which react with VSCs to form stable, non-volatile metal sulfides (Young et al., 2003). Additionally, antimicrobial agents are used to reduce the bacterial load, effectively lowering the production of these compounds at the source.

Mechanism of action

Therapeutic intervention involves the chemical neutralization of VSCs into non-volatile salts (e.g., zinc-sulfur complexes), the oxidation of sulfur groups (e.g., by chlorine dioxide), or the reduction of the anaerobic bacterial population responsible for their synthesis via antimicrobial action.
