Botsford Health Care Continuum was a comprehensive healthcare provider network based in Farmington Hills, Michigan, specializing in post-acute and long-term care services. As a division of Botsford Health Care, it offered a wide range of services including skilled nursing, sub-acute rehabilitation, assisted living, and home health care. The organization was designed to provide a seamless transition for patients moving from acute hospital settings to recovery or residential care. In 2014, Botsford Health Care merged with Beaumont Health System and Oakwood Healthcare to form Beaumont Health. Following a subsequent merger in 2022, the entity became part of Corewell Health. Today, its legacy facilities continue to serve the Southeast Michigan community under the Corewell Health brand.
