CenseoHealth was a U.S. healthcare services company focused on prospective health assessments and Annual Wellness Visit-compliant evaluations for health plans and healthcare delivery organizations. The company built a large contracted network of physicians to perform in-home, office-based, and community-based assessments, with the goal of improving member engagement, identifying unmet clinical needs, and supporting care management. Its business was centered on Medicare Advantage and other payer-oriented risk assessment workflows rather than drug development. In late 2017, CenseoHealth was acquired in a New Mountain Capital-backed transaction and combined with Advance Health; the combined business was later launched publicly as Signify Health in 2018. As a result, CenseoHealth no longer operates as an independent active company.
