Central New York Care Collaborative

Central New York Care Collaborative (CNYCC) is a non-profit organization established to lead the implementation of the Delivery System Reform Incentive Payment (DSRIP) program in the Central New York region. The collaborative connects a vast network of over 2,000 healthcare and community-based providers across six counties, including Onondaga, Oswego, and Cayuga. Its primary mission is to transform the healthcare delivery system for Medicaid and uninsured populations by shifting from volume-based to value-based care models. CNYCC focuses on improving population health outcomes through integrated behavioral health, chronic disease management, and enhanced care coordination. The organization provides data analytics, clinical support, and workforce training to its partner members to ensure sustainable healthcare improvements.
