Centura Home Health

Centura Health at Home was the home-based care division of Centura Health, providing skilled nursing, rehabilitation, and hospice services across Colorado and western Kansas. The organization specialized in transitional care, helping patients manage chronic conditions and recover from surgeries within their own homes. In 2023, following the dissolution of the Centura Health joint venture, the home health operations were transitioned to the individual management of CommonSpirit Health and AdventHealth. Today, these services continue under the respective brands of the parent organizations, focusing on integrated clinical support and patient-centered home care. The division was known for its focus on reducing hospital readmissions through comprehensive at-home clinical monitoring.
