The Champlain Local Health Integration Network (LHIN) was a regional health authority responsible for planning, funding, and integrating healthcare services in Eastern Ontario, Canada. Serving a population of over one million residents, the organization oversaw the performance of hospitals, long-term care homes, and community health centers. In addition to its administrative role, the Champlain LHIN directly managed home and community care services for patients requiring support in their residences. Following provincial healthcare reforms initiated in 2019, its planning and funding functions were absorbed into Ontario Health, while its service delivery arm was transitioned to Home and Community Care Support Services.
