Choose Healthy Life

Choose Healthy Life is a U.S.-based nonprofit healthcare organization focused on reducing health disparities in underserved Black communities by partnering with Black churches. Its operating model places trained faith-based health navigators in churches to connect community members with free health screenings, vaccinations, wellness programming, and health insurance enrollment support. The organization has worked with partners including Quest Diagnostics and United Way of New York City and has expanded from an initial COVID-19 response effort into a broader health equity and preventive care platform. Rather than developing therapeutics, Choose Healthy Life delivers community-based public health services and care access programs at scale across multiple states.
