Cognify

Cognify was a U.S. healthcare technology company focused on integrated care coordination and financial management software for provider organizations, particularly those serving frail elderly populations. Its products and services supported patient-centered, multidisciplinary care delivery for Program of All-inclusive Care for the Elderly (PACE), managed long-term care, and medical home providers. The company appears to have operated primarily as a health IT and enterprise software business rather than a therapeutics developer, with no evidence of a drug pipeline or clinical-stage assets. Cognify was founded in 1998 and later acquired by Tabula Rasa HealthCare in 2018. Following that acquisition, Cognify appears to have ceased operating as an independent company.
