Community Care Physicians

Community Care Physicians (CCP) is the largest independent multi-specialty medical group in the Capital Region of New York. Founded in 1984, the organization provides a comprehensive range of healthcare services across more than 30 specialties, including primary care, oncology, neurology, and urgent care. CCP operates a physician-led model with over 1,900 employees and 420 practitioners serving patients in multiple counties. The group is known for its adoption of advanced health information technology, including a centralized electronic health record system and a robust patient portal. In 2022, CCP entered into a strategic partnership with the health insurer CDPHP to form an integrated delivery system and a joint management services organization to streamline administrative functions.
