Community Care Plan is a Florida-based managed care health plan founded in 2000 and owned by Broward Health and Memorial Healthcare System. The organization provides commercialized health coverage and care-management services rather than developing therapeutic products, with offerings spanning Florida Medicaid Managed Medical Assistance, Florida Healthy Kids, and self-insured employee health plans. It also operates SydCura, a division focused on performance and value-based care programs for uninsured, uncompensated, and underserved populations, and has launched a health insurance marketplace division called 22 Health. Community Care Plan primarily serves members in Florida through provider-network, benefits administration, and population-health management services.
