Community Health Net

Community Health Net is a nonprofit Federally Qualified Health Center based in Erie, Pennsylvania, providing community-based medical, dental, vision, behavioral health, sports medicine, and pharmacy services. Founded in 1985, it began as Primary Health Care Services of Northwestern PA and has grown into a regional safety-net healthcare provider with multiple service locations.
