Community LIFE

Community LIFE is a non-profit Program of All-Inclusive Care for the Elderly (PACE) dedicated to helping seniors live independently in their own homes. The organization provides a comprehensive suite of healthcare and social services, including primary medical care, home health services, physical and occupational therapy, and prescription drug coverage. By utilizing an interdisciplinary team of professionals, Community LIFE coordinates all aspects of a participant's care, focusing on preventative health and long-term wellness for the elderly population in Western Pennsylvania. It operates several adult day centers that offer social engagement and nutritional support alongside clinical treatment. The program serves as a specialized alternative to nursing home placement for eligible individuals.
