EngageWell IPA (Independent Practice Association) is a New York City-based healthcare network formed by not-for-profit human services agencies to deliver integrated, value-based care. The network coordinates primary care, behavioral health, substance use treatment, harm reduction, and social determinants of health (such as housing and nutrition) for low-income and marginalized populations, including those living with HIV/AIDS. Through its flagship 'Care Your Way' initiative, EngageWell leverages telehealth, remote patient monitoring, and community health workers to bridge gaps in care. The organization operates in partnership with digital health platforms and managed care organizations to improve health outcomes and advance health equity.
