Family Health Network (FHN) was a prominent Chicago-based, not-for-profit managed care organization that specialized in providing Medicaid health plans to residents of Cook County, Illinois. Established in 1995 by a consortium of local hospitals, the company focused on community-based healthcare delivery and improving access for underserved populations. FHN operated as a provider-sponsored health plan, emphasizing care coordination and local provider partnerships with safety-net hospitals. In 2018, the organization was acquired by Cook County Health and its members were transitioned into the CountyCare health plan. This acquisition made CountyCare the largest Medicaid managed care plan in the region and marked the end of FHN's independent operations.
