Foundations Family Medicine

Foundations Family Medicine is a primary care medical practice based in southern Indiana, with offices in Austin and Henryville. Established in 2004 by Dr. William Cooke, the practice became a critical healthcare provider during the 2015 HIV and opioid crisis in Scott County. The clinic offers comprehensive services including family medicine, prenatal care, behavioral health, and specialized treatment for infectious diseases like HIV and Hepatitis C. In January 2023, the practice was acquired by Well Care Community Health, a Federally Qualified Health Center (FQHC). It now operates as a division of Well Care, maintaining its mission to provide evidence-based and affordable healthcare to rural and underserved communities. The facility is recognized for its integrated approach to primary care and substance use recovery support.
