Fox Insurance Company was a private health insurance firm that specialized in providing Medicare Part D prescription drug coverage to beneficiaries across the United States. The company is primarily known for a significant regulatory event in March 2010, when the Centers for Medicare & Medicaid Services (CMS) issued an immediate termination of its contract—a rare and severe enforcement action. CMS cited the company's systemic failure to provide beneficiaries with access to life-sustaining medications and an unacceptably high rate of improper coverage denials as the primary reasons for the termination. At the time of its closure, Fox Insurance served approximately 123,000 members in 23 states and the District of Columbia. Following the loss of its federal contract, the company was unable to sustain operations and subsequently entered liquidation.
