Hackensack Alliance ACO, officially known as Hackensack Physician-Hospital Alliance ACO, LLC, was an Accountable Care Organization based in New Jersey and a subsidiary of Hackensack Meridian Health. Established in 2012, it was one of the first 27 organizations in the nation to participate in the Medicare Shared Savings Program (MSSP). The organization operated as a network of physicians and hospitals dedicated to improving the quality of care for Medicare beneficiaries while reducing healthcare costs through better coordination and evidence-based medicine. Over its decade-long history, it consistently achieved high quality scores and generated over $200 million in total savings for the Medicare program. In 2023, the entity was consolidated into the unified HMH ACO to centralize population health management across the entire Hackensack Meridian Health network.
