Health Partners Plan

Jefferson Health Plans, formerly known as Health Partners Plans (HPP), is a non-profit managed care organization headquartered in Philadelphia, Pennsylvania. Founded in 1984, the company provides a variety of health insurance options, including Medicaid, Medicare, and Children's Health Insurance Program (CHIP) plans. It primarily serves members in Southeastern Pennsylvania and is noted for its focus on community health and addressing social determinants of health. In 2021, the organization became a wholly-owned subsidiary of Jefferson Health, leading to its official rebranding in late 2023 to better integrate with the parent health system's clinical and academic resources.
