Health Share of Oregon

Health Share of Oregon is a non-profit Coordinated Care Organization (CCO) that serves Oregon Health Plan (Medicaid) members in Clackamas, Multnomah, and Washington counties. Established in 2012, it operates as a unique collaborative partnership between local governments, health systems, and health plans to integrate physical, dental, and mental health services. The organization is the largest CCO in Oregon, managing care for over 400,000 individuals through a comprehensive network of providers. Its primary mission is to improve the health of its members and the community while containing costs through innovative care coordination and social determinants of health initiatives. Health Share focuses on community-based health solutions and health equity to ensure high-quality care for the region's most vulnerable populations.
