Heartland Alliance Health is a Chicago-based nonprofit community health organization and federally qualified health center that provides primary medical, dental, mental health, substance use, and outreach services. Its care model is focused on underserved populations, including people experiencing homelessness, and extends beyond fixed clinics into shelters, transitional housing programs, encampments, and street outreach. The organization operates health centers in Uptown and Englewood and also provides medical case management and food pantry support. Formerly part of Heartland Alliance, it became an independent nonprofit during the parent organization’s 2024 restructuring. After announcing a planned shutdown in February 2025, the organization later stated it would remain open following emergency philanthropic support.
