JaxCare was a provider-sponsored Health Maintenance Organization (HMO) based in Jacksonville, Florida, that specialized in providing managed care services to Medicaid beneficiaries. Founded in 1995 by a coalition of local healthcare providers, including the Duval County Medical Society, the company was designed to improve health outcomes for underserved populations through a coordinated 'medical home' approach. JaxCare operated primarily in the Duval County area, focusing on aligning provider incentives with patient care quality. In 2009, the company was acquired by WellCare Health Plans, Inc., a leading provider of government-sponsored managed care services, to strengthen WellCare's Medicaid presence in the Florida market. Following the acquisition, JaxCare's operations and provider networks were integrated into WellCare's broader Florida Medicaid business.
