Journey Home Hospice

Journey Home Hospice is a specialized healthcare facility providing 24/7 hospice and palliative care specifically for individuals experiencing homelessness and structural vulnerability. Launched in 2018 as a program of the Saint Elizabeth Foundation (SE Health), it operates sites in Toronto and Windsor, Ontario. The hospice employs an interdisciplinary team of physicians, nurses, and social workers trained in trauma-informed and harm-reduction approaches. It serves as a unique model for equitable end-of-life care, offering services at no cost to patients through community support and government funding. The facility also functions as a teaching and learning center for healthcare providers to study specialized palliative care delivery. It is the first hospice in Toronto dedicated to this specific population.
