Kool Smiles was a prominent American dental service organization (DSO) that specialized in providing pediatric and general dental care, primarily targeting underserved communities and Medicaid recipients. Founded in 2002, the company operated over 120 affiliated clinics across 15 states at its peak. The brand became the subject of significant legal and regulatory scrutiny regarding its billing practices and the clinical necessity of treatments, leading to a $23.9 million settlement with the U.S. Department of Justice in 2018. Following these challenges and a subsequent bankruptcy filing by its parent company, Benevis, the Kool Smiles brand was retired and most locations were rebranded. The clinics now operate under various local names managed by the Benevis corporate entity.
