Legacy Community Health

Legacy Community Health is a Texas-based nonprofit community health system and the largest Federally Qualified Health Center in Texas. The organization operates more than 50 care locations across the Texas Gulf Coast and provides adult and senior primary care, pediatrics, OB/GYN, behavioral health, dental, HIV/AIDS care, vision, specialty care, pharmacy services, and school-based health programs. Legacy was formed in 2005 through the merger of Montrose Clinic and The Assistance Fund, with legal roots dating to 1981.
