The Los Angeles County Community Health Plan (CHP) was a public health maintenance organization operated by the Los Angeles County Department of Health Services. Established to provide managed care services to low-income populations, it served residents enrolled in Medi-Cal, Healthy Families, and other government-sponsored programs. The plan functioned as a "Plan Partner" for L.A. Care Health Plan, offering members access to the County's extensive network of public hospitals and clinics. In 2014, the Los Angeles County Board of Supervisors voted to dissolve the plan's independent HMO status and transition its members to other managed care partners. This restructuring was part of a broader effort to improve the financial sustainability and care coordination of the County's health delivery system.
