M-Plan was a prominent health maintenance organization (HMO) based in Indianapolis, Indiana, that served as one of the state's largest managed care providers. Founded as a joint venture between Clarian Health Partners (now IU Health) and Methodist Hospital, the company provided comprehensive health insurance and medical management services to over 125,000 members.
